Please fill out our new patient registration forms listed down below. You must print and complete the necessary forms before we schedule you an appointment. For any questions, please call the office.

Please note, this email is strictly for registrations forms. Established patients should be using the MyChart (Patient Portal) through John Muir to send emails or upload medical records.
Accepted Insurance Providers
Gilroy Family Medical Group works with most major insurances. We understand insurance plans are complex and may be difficult to understand. We accept Medicare and most PPO/HMO plans. If you have any questions, please call our office and one of our knowledgeable staff members would be happy to help. We also recommend you checking with your insurance if Bay Area Surgical Specialist comes up as in network with the current plan you have selected.

Please note, we DO NOT accept Medi-cal or Kaiser Insurance. If you have an HMO plan make the appropriate changes to reflect the physician you have selected to see in our office.
